Аннотация: Представлений опис випадку рідкісного захворювання – прогресуючої атрофії обличчя у пацієнтки неврологічного відділення Полтавської обласної клінічної лікарні. Проведений огляд літератури щодо проблеми, проаналізовані етіологічні чинники, патогенез та клінічні характеристики захворювання. Визначене коло необхідних методів обстеження хворих, яке включає огляд невролога, ревматолога, ендокринолога, а також використання додаткових інструментальних і лабораторних методів дослідження. Комплексна лікувальна тактика передбачає використання трофотропних, судинних засобів, анаболічних гормонів, препаратів калію, масаж, ЛФК.

Аннотация: У статті запропоновано метод полімеразної ланцюгової реакції довжин рестрикційних фрагментів для виявлення однонуклеотидного поліморфізму (rs2583988) для подальшого прогнозування ризику хвороби Паркінсона. На підставі результатів мета аналізу для однонуклеотидного поліморфізму rs2583988 (C більше T) в гені альфа-синуклеїна. Поліморфний нуклеотид був обраний шляхом аналізу біоінформатичних та генетичних досліджень. Ми розробили пару праймерів та умови рестрикційного аналізу для виявлення різних алелів у rs2583988. Як результат, був розроблений простий і доступний метод виявлення однонуклеотидного поліморфізму алелів rs2583988. Очікується, що метод виявиться зручним для клінічних лабораторій, завдяки доступній вартості та обладнанню, а також використовуватиметься в подальших асоціативних дослідженнях.

Аннотация: Levodopa is the gold standard for the treatment of Parkinson’s disease. Motor fluctuations and dyskinesia are important complications of levodopa therapy that affect many patients with Parkinson’s disease. The aim of our study was to determine the nature of motor fluctuations and optimize the treatment approach for various forms of Parkinson’s disease. We examined 80 patients with Parkinson’s disease who were undergoing examination and inpatient treatment in the neurological department of the Poltava Regional Clinical Hospital named after M.V. Sklifosovsky and on the basis of the Center for patients with Parkinson’s disease and other neurodegenerative diseases on the basis of the Department of Nervous Diseases of the Poltava State Medical University. All patients included in the study were treated with levodopa drugs. Depending on levodopa therapy, patients were divided into subgroups: a subgroup – patients received therapy with short-acting levodopa drugs 4 times a day. b subgroup – patients received combined short-acting levodopa 3 times per day with extended-acting levodopa before sleep.The patients with akinetiс-rigid form has more severe motor disturbances and lower level of independence. These differences cause finding method for optimization of treatment with considering motor forms of Parkinson’s disease. Among the motor complications encountered in patients with various forms of Parkinson’s disease, the following prevailed: the phenomenon of “wearing out”, unpredictable periods of disconnection, freezing, insufficient “switching on”. It was shown that in patients with rigidity form combined therapy significantly decreased duration “off” period, improved UPDRS-III) and Schwab and England scores, when in group with akinetic-rigidity-tremor form it only decreases UPDRS-III score. In the course of the treatment, a significant improvement in the clinical symptoms and reduction of motor complications were observed in Parkinson’s disease patients receiving treatment using combined levodopa therapy (a combination of short-acting levodopa and long-acting levodopa). we have shown better response for changing treatment strategy in patients with rigidity forms of disease. We can recommend to consider adding of long-release levodopa for management of motor complications in Parkinson’s disease patients with rigidity and akinetic-rigid and tremor-dominant forms of Parkinson’s disease.
Леводопа є золотим стандартом лікування хвороби Паркінсона. Рухові флуктуації та дискінезія є важливими ускладненнями терапії леводопою, які вражають багатьох пацієнтів із хворобою Паркінсона. Метою нашого дослідження було визначити характер рухових флуктуацій та оптимізувати тактику лікування різних форм хвороби Паркінсона. Обстежено 80 хворих на хворобу Паркінсона, які перебували на обстеженні та стаціонарному лікуванні в неврологічному відділенні Полтавської обласної клінічної лікарні імені М.В. Скліфосовського та на базі Центру для хворих на хворобу Паркінсона та інші нейродегенеративні захворювання на базі кафедри нервових хвороб Полтавського державного медичного університету. Усі пацієнти, включені в дослідження, отримували лікування препаратами леводопи. Залежно від терапії леводопою пацієнти були розподілені на підгрупи: підгрупа – пацієнти отримували терапію препаратами леводопи короткої дії 4 рази на добу. б підгрупа – пацієнти отримували перед сном комбінацію леводопи короткої дії 3 рази на добу з леводопою пролонгованої дії. У хворих з акінетико-ригідною формою виражені рухові порушення та нижчий рівень самостійності. Ці відмінності зумовлюють пошук методу оптимізації лікування з урахуванням рухових форм хвороби Паркінсона. Серед рухових ускладнень, що зустрічаються у хворих на різні форми хвороби Паркінсона, переважали: феномен «зношування», непередбачувані періоди виключення, завмирання, недостатнє «включення». Показано, що у хворих із акінетико-ригідною формою комбінована терапія достовірно зменшує тривалість періоду «вимкнення», покращує показники (UPDRS-III) та Шваба та Інгланда, тоді як у групі з акінетикоригідно-тремтячою формою лише знижує показник UPDRS-III. У процесі лікування у хворих на хворобу Паркінсона, які отримували комбіновану терапію леводопою (комбінація леводопи короткої дії та леводопи тривалої дії), спостерігалося значне покращення клінічних симптомів та зменшення моторних ускладнень. ми показали кращу відповідь на зміну стратегії лікування у пацієнтів з ригідними формами захворювання. Ми можемо рекомендувати розглянути можливість додавання леводопи тривалого вивільнення для лікування рухових ускладнень у пацієнтів із хворобою Паркінсона з ригідністю та акінетико-ригідно -тремтячою формами хвороби Паркінсона.

Аннотация: Хвороба Паркінсона характеризується дофамінергічним дегенеративним процесом, що вражає нейрони чорної субстанції. Захворювання в основному розглядається як руховий розлад, але пацієнти страждають не лише від моторних симптомів, а й від немоторних симптомів, які також є поширеними та можуть значно впливати на якість життя пацієнтів. Ці ускладнення включають когнітивні, психіатричні, вегетативні розлади, розлади сну та сенсорні розлади. Немоторні симптоми можуть з’явитися ще до появи перших моторних симптомів, але вони більш неприємні на пізніших стадіях ХП, коли можуть стати серйозними проблемами для пацієнтів і часто становлять виклик для лікуючих лікарів. Метою нашого дослідження було оцінити вплив оптимізації терапії леводопою у пацієнтів із ХП та руховими флуктуаціями на вираженість немоторних симптомів. Було обстежено 80 хворих на різні форми ХП із Центру хворих на ХП та інші нейродегенеративні захворювання кафедри неврологічних захворювань Полтавського державного медичного університету. Пацієнти, залежно від поширеності симптомів та результатів електронейроміографічного дослідження, були розподілені на групи. Клінічні оцінки проводили через 3 місяці після зміни режиму лікування леводопою. Збирали дані про вік, стать пацієнтів, тривалість захворювання та локалізацію дебюту захворювання, досліджували тяжкість хвороби за шкалою UPDRS, інтенсивність та рівень немоторних симптомів – за шкалою немоторних симптомів (NMSS). Було встановлено, що оптимізація лікування шляхом зміни режиму прийому леводопи може значно покращити рухову функцію. Відсутність суттєвих відмінностей за іншими субіндексами UPDRS можна було пояснити фрагментацією питань, пов’язаних з немоторними симптомами, між різними доменами. Поліпшення рухової функції при застосуванні леводопи пролонгованої дії було пов’язане зі зменшенням вираженості моторних флуктуацій. Встановлено, що застосування пролонгованої форми леводопи під час вечірнього прийому супроводжується достовірним зниженням вираженості за шкалами «серцево-судинні та падіння», «увага/пам’ять», «шлунково-кишковий тракт» та за сумарним балом NMSS у групі пацієнтів із акінетико-ригідною формою захорювання. Таким чином, нами встановлено, що лікування пацієнтів із різними формами ХП препаратом леводопа асоціюється з широким спектром немоторних симптомів, які більш виражені при акінетико-ригідній формі ХП. Додавання ретардних форм леводопи може покращити вираженість моторних флуктуацій і зменшити інтенсивність немоторних

Аннотация: Хвороба Паркінсона характеризується розвитком моторних симптомів, зокрема брадикінезії, м’язової ригідності та тремору, а також немоторних симптомів, зокрема порушень сну, настрою, серцево-судинної та травної системи і сенсорні розлади. Моторні та сенсорні симптоми зазвичай при хворобі Паркінсона аналізуються за клінічними шкалами з суб’єктивними оцінками. Тому необхідний розвиток нових технік та оптимізація об’єктивних методик у клінічній практиці. Нами проведено крос-секційне аналітичне дослідження на базі центру хвороби Паркінсона та інших нейродегенеративних захворювань при Полтавському державному медичному університеті у період 2020–2021 рр. Нами виконано факторний аналіз електронейроміографічних показників. Було визначено показники, які характеризують тремор та тяжкість карпального і кубітального тунельних синдромів у пацієнтів з хворобою Паркінсона. Так використання факторних навантажень, встановлених за результатами нашого дослідження, дозволяють оптимізувати діагностичні підходи до периферичних уражень при хворобі Паркінсона та розрахувати тяжкість тунельних нейропатій і вираженість тремору.
