Аннотация: Цель работы – повышение эффективности лечения нарушений сна у пациентов с артериальной гипертензией (АГ) и ожирением. Материалы и методы. Обследованы 62 больных (средний возраст – 58,3 ± 2,3 лет) с АГ и ожирением. Всем обследованным проведено общеклиническое обследование, суточный мониторинг АД, оценка качества жизни (опросник «General Well – Being Questionnaire»). Для выявления нарушений сна (диссомния) использовали критерии МКРС-2 (2005), а оценки нарушений циркадного ритма «сон – бодрствование» – анкеты балльной оценки субъективных характеристик сна. У пациентов обеих групп в исходных условиях отмечены нарушения сна. После регистрации исходных данных пациентам назначали базисную терапию, из них 32 (1 группа) дополнительно получала препарат мелатонин в дозе 3 мг в течение 4 недель; 30 пациентов составили 2 группу. Результаты. После лечения в 1 группе уровни офисного САД и ДАД были на 6,9 % и на 6,7 % (p 0,05), а среднесуточные САД и ДАД (по данным СМАД) – на 7,9 % и 6,7 % (p 0,05) ниже, чем во 2 группе. У пациентов 1 группы отмечены позитивные изменения показателей липидного, углеводного обмена и достоверное улучшение субъективных характеристик сна и циркадного ритма «сон – бодрствование» наряду с улучшением показателей качества жизни. Выводы. Таким образом, назначение мелатонина пациентам с АГ и ожирением с нарушениями сна повышает эффективность гипотензивной терапии, положительно влияет на глюкометаболические параметры, субъективную оценку качества сна и качество жизни.

Аннотация: In 156 patients with stage II arterial hypertension, including 96 of them with diabetes mellitus type 2, the relationship between the relative telomeres length of blood leukocytes (RLTL) and buccal epithelium cells (RBTL) and different levels of carbohydrate metabolism have been determined and data in patients with arterial hypertension combined with diabetes mellitus type 2 and in patients with isolated hypertension have been compared. Carbohydrate metabolism parameters, blood pressure levels were evaluated. RLTL and RBTL were determined by a real time quantitative polymerase chain reaction. It is shown that a hypertension and diabetes mellitus type 2 combination was accompanied by greater significant RLTL (p=0.009) and RBTL (p=0.001) more than with isolated hypertension. In the patients with hypertension and diabetes mellitus type 2 the relative length of telomeres was assessed in the carbohydrate metabolism compensation and in its absence and no convincing data was obtained on the effect of glycemic control on the relative length of telomeres. In patients with isolated hypertension it was found that those with a lower glucose level than that within the risk factor (5.6-6.9 mmol/1) had a shorter mean relative telomeres length both of blood leukocytes (p=0.05) and the buccal epithelium cells (p=0.03) regarding indicators for those who had higher blood glucose levels. Patients with a comorbid course of hypertension and diabetes mellitus type 2 had more pronounced decrease in the RTL. Achieving glycemic control at hypertension and diabetes mellitus was accompanied by a significant increase in the RBTL. The relative length of blood leukocytes telomere was controversial. Additional determining the RBTL enhances the diagnostic and prognostic power when assessing the efficacy of correcting glycemic control in high cardiovascular risk patients.

Аннотация: Метою дослідження було підвищення ефективності лікування артеріальної гіпертензії (АГ) у осіб молодого віку з клінічними ознаками вегетативної дисфункції. Обстежено 65 хворих АГ чоловічої статі (середній вік 35,6 ± 4,3 років) з клінічними ознаками вегетативної дисфункції, яких 30 (1 група) призначалась базисна терапіяі 35 пацієнтам (2 група) додатково призначали етилметилгідроксипіридинасукцинат (ЕМС) протягом 4 тижнів. Після лікування корекція АТ до цільових значень була досягнута 76,7 % пацієнта 1-ї групи і 88,6% 2-ої груп. На фоні терапії ЕМС, спостерігався більш виражене зростання загальної ВРС, зниження активності симпатичного відділу вегетативної нервової системи і відновленню вегетативного балансу згідно даних тимчасових показників ВРС, а також суттєва позитивна динаміка ситуативної та особистої тривожності. Таким чином, включення ЕМС у комплексну терапію лікування чоловіків молодого віку з АГ і клінічними ознаками вегетативної дисфункції позитивно впливає на клінічну симптоматику, сприяє відновленню вегетативного балансу і корекції психоемоційного стану.

Аннотация: Метою дослідження було вивчення впливу фіксованої комбінації мельдонію дигідрату та ?-бутиробетаїну дигідрату (МД+?-БД) на клініко-гемодинамічні та глюкометаболічні параметри, а також показники оксидантно-антиоксидантного статусу у пацієнтів з хронічною серцевою недостатністю (ХСН) та цукровим діабетом (ЦД) 2 типу. Обстежено 62 хворих з ХСН та ЦД 2 типу. Після реєстрації вихідних даних усім пацієнтам призначали базисну терапію, з них 30 (1 група) додатково фіксовану комбінацію МД+?-БД по 2 капсули тричі на добу протягом 3 місяців та 32 пацієнта увійшли у групу порівняння (2 група). Після лікування в 1 групі визначалось збільшення пройденої дистанції за даними теста 6-хвилинної ходьби на 31,2% і 1,9%(p0,05) відповідно. Спостерігалось позитивні зміни глюкометаболічних показників. Відзначено зменшення активності процесів перекісного окислення ліпідів за рівнем малонового діальдегіду (?-22,3% у 1 групі, проти ?-11,2% у 2 групі, р0,05) і підвищення активності супероксиддисмутази еритроцитів (відповідно ?29,5% проти 8,1%, р0,05). Таким чином, додавання МД+?-БД до комплексної терапії пацієнтів з ХСН та ЦД 2 типу покращує клініко-гемодинамічні і глюкометаболічні показники та коригує дисбаланс оксидантно-антиоксидантної системи.

Аннотация: Метою дослідження було оцінити ефективність терапії з додаванням оротату магнію у пацієнтів з АГ і ЦД 2 типу з гіпомагніємією. Обстежено 60 хворих з АГ та ЦД 2 типу з гіпомагніємією. Після реєстрації вихідних даних усім пацієнтам призначалась базисна терапія, з яких 32 пацієнтам (1 група) додатково – оротат магнію протягом 8 тижнів і 28 пацієнтів склали групу порівняння. Встановлено, що антигіпертензивна ефективність терапії була більшою в 1 групі. Визначено більш виражену позитивну динаміку показників ліпідного обміну та вуглеводного профілю, показників якості життя у пацієнтів 1 групи. Таким чином, включення в терапевтичний комплекс оротату магнію пацієнтам з АГ і ЦД з гіпомагніємією підвищує ефективність гіпотензивної терапії, позитивно впливає на глюкометаболічні параметри та якість життя у цієї категорії хворих.

Аннотация: Метою роботи було вивчити динаміку параметрів діастолічної, функції, вмісту розчинного рецептора стимулюючого чинник зростання, що експресується геном 2 (sST2) і N-термінального фрагмента мозкового натрійуретичного пропептида (NT-proBNP) в крові у пацієнтів з артеріальною гіпертензією при додаванні до антигіпертензивної терапії еплеренону. Встановлено, що у пацієнтів з артеріальною гіпертензією 2 ступеня додавання еплеренону до антигіпертензивної терапії сприяє більш вираженому зниженню артеріального тиску, виміряного пацієнтами при самоконтролі; при добовому моніторингу порівняно з антигіпертензивною терапією. Включення селективного антагоніста мінералокортикоїдних рецепторів альдостерону еплеренону в комплексну терапію хворих на артеріальну гіпертензію 2 ступеня надає виражений позитивний вплив на структурно-геометричні показники міокарда, параметри діастолічної функції лівого шлуночка, а також вміст sST2 і NT-proBNP в сироватці крові поряд з хорошою переносимістю.

Аннотация: Обстежено 60 жінок віком (середній вік 62,7±3,8 років) з артеріальною гіпертензією II стадії, 2 ступеня та постменопаузальним остеопорозом за даними остеоденситометрії, які не приймали регулярну антигіпертензивну терапію. Після реєстрації вихідних даних 30 пацієнткам (1 група) призначали терапію артеріальної гіпертезії комбінацією лізиноприлу 10–20 мг та індапаміду 2,5 мг на добу та 30 пацієнткам (2 група) – лізиноприл 10–20 мг плюс гідрохлортіазид 12,5 мг на добу протягом 12 тижнів. Контрольну групу склали 20 практично здорових жінок. Усім обстеженим особам проведено клініко-лабораторне обстеження згідно протоколам обстеження пацієнтів з артеріальною гіпертезією та остеопорозом. Аналіз показників через 12 тижні виявив високу антигіпертензивну ефективність обох схем лікування. У пацієнток з артеріальною гіпертензією та постменопаузальним остеопрозом за даними офісного вимірювання артеріального тиску та добовим моніторингом артеріального тиску, 12-тижнева терапія комбінацією лізиноприлу плюс індапамід за вираженістю антигіпертензивного ефекту, здатності нормалізації добового профілю артеріального тиску, зниженню показників навантаження тиском достовірно не відрізнялася від терапії лізіноприлом у поєднанні з гідрохлортіазидом. Швидкість поширення пульсової хвилі та рівні центрального аортального тиску (цСАТ та цПТ) за даними аппланаційної тонометрії достовірно більше знижувалися на тлі 12-тижневої терапії лізиноприлом у поєднанні з індапамідом порівняно з комбінацією лізиноприлу з гідрохлортіазидом поряд з хорошою переносимістю в обох лікувальних групах.

ЛЕКАРСТВЕННАЯ ТЕРАПИЯ КОМБИНИРОВАННАЯ -- DRUG THERAPY, COMBINATION
Аннотация: Метою дослідження було дослідити вплив комплексної терапії з додаванням еплеренону та триметазидину на стан діастолічної функції лівого шлуночка, а також рівні розчинних ростостимулюючих рецепторів, експресованих геном 2 та N-кінцевим фрагментом мозкового натрійуретичного пропептиду у хворих на артеріальну гіпертензію з цукровим діабетом 2 типу. Після реєстрації вихідних даних 42 пацієнтам (група 1) було призначено базову терапію, а 38 пацієнтам (група 2) – еплеренон у дозі 25-50 мг один раз на добу та триметазидин у дозі 80 мг один раз на добу на додаток до антигіпертензивної терапії протягом 3 місяців. Контрольна група складалася з 20 практично здорових особин. Після проведеного лікування було виявлено значне зниження рівня розчинних ростостимулюючих рецепторів, експресованих геном 2, в крові пацієнтів обох груп, але в 2 групі цей показник не відрізнявся від контрольної, в той час як в 1 групі він залишався значно вище, ніж у здорових осіб. При цьому рівень N-кінцевого фрагмента мозкового натрійуретичного пропептиду значно знизився в 1,3 рази і в 1,6 рази в групах 1 і 2 відповідно, але залишився достовірно вище, ніж в контрольній групі. Достовірне зниження Е/е'середнього було визначено в обох групах хворих, але в 2 групі на 8,9 % менше, ніж у 2 групі, і зниження геометричних параметрів лівого передсердя, більш виражене в 2 групі. Таким чином, додавання еплеренону та триметазидину до комплексної терапії хворих на артеріальну гіпертензію та цукровий діабет 2 типу протягом 3 місяців покращувало діастолічну функцію, а також позитивно впливало на біомаркер стресу міокарда та фіброзу розчинних ростостимулюючих рецепторів, експресованих рівнем гена 2, та дисфункції міокарда N-кінцевий фрагмент головного мозку концентрація натрійуретичного пропептиду в сироватці крові.

Аннотация: Метою. роботи було вивчення особливостей стану діастолічної функції лівого шлуночка та фактору некрозу пухлин-? у пацієнтів з поєднаною патологією артеріальна гіпертензія з цукровим діабетом 2 типу та ожирінням. Встановлено, що у пацієнтів з артеріальною гіпертензією з цукровим діабетом 2 типу та ожирінням порівняно з групою пацієнтів з артеріальною гіпертензією і цукровим діабетом 2 типу з нормальною масою тіла частіше реєстрували порушення діастолічної функції лівого шлуночка. Визначено достовірне підвищення вмісту фактору некрозу пухлин-? у групі пацієнтів з артеріальною гіпертензією з цукровим діабетом 2 типу та ожирінням у порівнянні з групою з артеріальною гіпертензією та цукровим діабетом 2 типу з нормальною масою тіла.
