Аннотация: В останні десятиліття значне місце у відновленні кровозабезпечення відіграють ендоваскулярні методи хірургічного лікування. Однак, залишаються недостатньо вивченими питання ускладнень, які виникають після проведення таких втручань, причини їх виникнення, які морфологічні зміни відбуваються у атеросклеротично зміненій стінці артерії при їх проведені. Метою дослідження було дати морфологічне обґрунтування застосування поетапної дозованої балонної ангіопластики з використанням балонів різного діаметру і довжини у порівнянні з стандартною методикою у хворих з ішемічною формою синдрому діабетичної стопи. Згідно отриманих результатів експериментальних досліджень встановлено, що в основному при виконанні поетапної дозованої ангіопластики за запропонованим способом внутрішня еластична перетинка артерії чітко виражена, має не значні ділянки фрагментації. При цьому, зовнішня еластична перетинка виражена достатньо добре на всьому протязі, має не значні ділянки фрагментації і їх було значно менше чим у тих випадках коли виконувалась ангіопластика за стандартною методикою. Причому, у зовнішній оболонці, де знаходилися судини судин та нерви судин, вони збереглися майже без змін. Зменшення кількості і величини розшарувань оболонок артерії, їх фрагментації при виконанні поетапної дозованої балонної ангіопластики за запропонованим способом з використанням балонів різного діаметру і довжини дозволяє рекомендувати її використання у практичній діяльності для зменшення частоти виникнення тромтичних ускладнень у ранньому післяопераційному періоді.

Аннотация: Клінічний перебіг синдрому діабетичної стопи негативно впливає на якість життя пацієнтів. Оцінка якості життя допоможе забезпечити кращий догляд та лікування хворих з такою патологією та зменшить вірогідність виникнення важких ускладнень цукрового діабету. Метою даного дослідження було оцінити якість життя пацієнтів з ішемічною формою синдрому діабетичної стопи після проведення ендоваскулярного лікування різними методами. Пацієнти з ішемічною формою синдрому діабетичної стопи у залежності від методу проведення балонної ангіопластики оклюзійно-стенотичних уражень підколінних і гомілкових артерій були розподілені на дві групи дослідження. Оцінку якості життя хворих проводили за допомогою опитувальника SF-36 перед операцією, через 7-10 діб та 6 місяців після неї. Всі етапи опитування протягом 6 місяців спостереження пройшли 38 (65.5 %) пацієнтів першої і 34 (63.0 %) хворих другої груп, які були включеними у дане дослідження. Отримані дані, дають змогу стверджувати, що проведення балонної ангіопластики підколінних і гомілкових артерій у хворих з ішемічною формою синдрому діабетичної стопи позитивно впливає на суб’єктивну оцінку пацієнтами свого фізичного та психічного стану, як безпосередньо після проведення таких операцій, так і через 6 місяців після їх виконання.
